Medical Record Request Policy

Please give 24 hours notice before picking up your records by calling our Central Medical Records Department at 813-875-7424. Our medical records are securely kept off site and therefore must be retrieved before we can sign them out to you or your doctor. We require a photo ID be available at the time of pick up.  If you would like to submit a reqeust for medical records, please click on the following link and one of our medical record associates will contact you to make all the appropriate arrangements:

Film printing fees:

• Film fee is determined by exam 
• Copying images to a CD will be free of charge
